Support at Home Program
Hope & Care Community Services supports older people to remain safe, independent and connected while living at home. Services are tailored to each person’s assessed needs, preferences, care plan and available Support at Home funding.
Support at Home Services
Depending on individual needs and service availability, support may include:
- Personal care and daily routines
- Cleaning, laundry, and household assistance
- Meal preparation and shopping
- Transport and community access
- Social support and companionship
- Respite care
- Nursing and allied health support
- Care management and service coordination
Who Can Access Support at Home?
The program is designed for older people who need coordinated assistance to continue living independently at home. Eligibility and approved services are determined through an aged care assessment.
The Support at Home program includes clinical supports, independence services and everyday living assistance. It replaced the Home Care Packages Program and Short-Term Restorative Care Programme on 1 November 2025.
Personalised Home Care
Following approval and funding allocation, participants can select a provider and agree on services that meet their assessed needs and available budget. Service arrangements can be reviewed when needs or circumstances change.
