NDIS & Disability Support

Hope & Care Community Services is a registered NDIS provider in Brisbane delivering a full range of disability supports. We work with NDIS participants of all ages and disability types across Brisbane South, Brisbane North, and Brisbane South West — helping people live independently, engage with their communities, and achieve their NDIS goals.

We work with the Department of Children, Youth Justice and Multicultural Affairs (DCSSDS), Queensland Youth Justice, and family support agencies to provide 24/7 care for young people aged 12 to 25.

Hope & Care Community Services provides coordinated transition and reintegration support to help people move safely from hospital — or a youth residential or detention facility — back into the community. We work with Queensland Health hospital social work teams, NDIA Health Liaison Officers, discharge planners, and DCSSDS to ensure supports are in place from day one.
