Hope & Care Community Services delivers community programmes and group activities across Brisbane, serving people with disability, older Australians, young people transitioning from residential care, and individuals re-entering the community after a period in hospital or a youth justice setting.

What is Included?
- Life Skills Workshops — budgeting, cooking, digital literacy, and employment readiness
- Social Inclusion and Wellbeing Groups — connection, peer support, and community engagement
- Youth Reintegration Programmes — supporting young people transitioning from residential care or youth justice settings
- Community Advocacy — helping individuals access housing, health, employment, and welfare services
- Post-Discharge Reintegration — supporting people re-entering the community after hospital or residential facility discharge

How to Refer?
Referrals accepted from DCSSDS, Queensland Youth Justice, Queensland Health, NDIA, GPs, hospital social workers, support coordinators, and other NDIS providers.
