Hope & Care Community Services provides short-term support for older people returning home after a hospital stay. Our service focuses on recovery, safety and rebuilding confidence with everyday activities.
Transition Care We Provide
Support may include:
- Personal care and daily routine support
- Meal preparation and household assistance
- Mobility and transfer support
- Transport to medical appointments
- Medication assistance within the worker’s role
- Coordination with health and community services
- Support to regain everyday living skills
Who Can Access Transition Care?
Post-hospital transition care may support older people who are medically ready to leave hospital but require short-term assistance during recovery. Government-funded Transition Care Program services require an assessment and approval before discharge from hospital.
Supporting Recovery at Home
Transition care is goal-oriented and may be provided at home, in the community, in an aged care setting or through a combination of locations. The Australian Government Transition Care Program generally provides short-term support for up to 12 weeks to help eligible older people regain independence and potentially avoid premature entry into longer-term care.
